Practice Notes
Welcome to the Journal

Nothing written here is a substitute for a clinical evaluation. Medicine is individual, and an article — however carefully written — cannot replace the conversation that happens when a physician knows your history, examines you, and looks at your test results. The journal is a starting point for thinking, not a prescription.
What you'll find here
Articles answering the questions patients actually ask in the consultation room. Things like: Do I really need this medication long-term? What does my ApoB number mean? Is my reflux something to worry about? How do I train for longevity without wrecking my joints?
Each piece is written to be useful on its own. Read one article and you should walk away with a clearer sense of what's going on, what a thoughtful physician would evaluate, and when a clinical visit is worth your time.
How it's written
The medicine here reflects how Dr. Birky practices day to day in Dubai Healthcare City — board-certified American family medicine, with attention to the longer-arc questions that fall under functional, longevity, and executive health work.
That means a few things in practice. Recommendations are anchored in published guidelines and the evidence behind them. Numbers, dosing, and reference ranges are specific where specifics help. Where the evidence is genuinely uncertain, that uncertainty is named rather than smoothed over. And the writing assumes you are an intelligent adult who wants to understand your own health, not a patient who needs to be talked down to.
You will not find marketing language, miracle protocols, or supplement stacks dressed up as science. You will also not find the kind of generic "wellness" content that could have been written by anyone, anywhere, about anyone.
Who it's for
The reader in mind is usually a busy professional — often in Dubai or elsewhere in the GCC — who is trying to make a good decision about their own health or someone they care about. Maybe you have already seen a physician and want a second perspective. Maybe you are deciding whether a particular test, medication, or specialist is worth pursuing. Maybe you just want to understand what is actually happening in your body before the next appointment.
Whatever brought you here, the goal of every article is the same: to leave you better informed than you arrived, and clearer about what to do next.
A note on what this isn't
Nothing written here is a substitute for a clinical evaluation. Medicine is individual, and an article — however carefully written — cannot replace the conversation that happens when a physician knows your history, examines you, and looks at your labs. The journal is a starting point for thinking, not a prescription.
